In the year 2012, on July 30th Notaro was diagnosed with cancer in both breasts, and during her live stage show at Largo In Los Angeles, she addressed her cancer diagnosis and other personal difficulties. The set was described as “instantly Legendary” with many comedians praising her work. She later released the audio with the booklet on iTunes.Live ended up selling more copies than other albums.

Notaro had a double mastectomy with no reconstructive surgery. she opted out of Chemo but decided to continue treatment with hormone-blocking.In November 2014 she showed her scars and through the force of her showmanship made her forget that they were there during the New York Comedy Festival it was a powerful and even inspiring, statement about survival and recovery and yet had a larky feel of a dare. Notaro was then hospitalized after a show in Philadelphia, and she required surgery for a  cyst. She then adopted a Vegan diet to eliminate the chronic pain she experienced in the years following her cancer diagnosis.
